You can install the Formesign - Fillable PDF addon for Google Forms to generate customized PDF documents from Google Forms response using Google Docs as template. You can also choose to show this as a consent page before submit.
Open your form in Google Forms > click on the ⋮ More icon next to the profile picture in the top right corner > click Get add-ons > search for Formesign - Fillable PDF > click on the Formesign - Fillable PDF addon to view the listing > click Install.
Click here to view the Formesign - Fillable PDF addon listing in Google Workspace marketplace > click Install. Once you have installed the addon, open your form in Google Forms > click on the addon icon > click Formesign - Fillable PDF to access the addon. You can use the addon to:
Show response summary for review before submit
Show a consent screen before submit
Generate customized PDF documents from Google Forms
Note: If you have logged in with multiple google accounts in your browser, sometimes the Google Workspace addons may not be installed properly. Open an incognito window in your Chrome browser, login with the preferred google account and the try to install the addon.
